Job Description :
Job Description Summary: This position manages the assigned area(s) and develops effective and compliant processes across Logan Health Medical Center (LHMC) related to inpatient case management.

Qualifications:

Minimum of one (1) of the following required:

BSN
Current Montana RN license or a multi state compact license with authorization to practice nursing within the state of Montana.
Masters of Social Work (MSW)

Case Management certification preferred and must be successfully obtained within two (2) years of hire.
Minimum of five (5) years’ acute care case management experience required. Tertiary care center and/or health system experience preferred.
Demonstrated knowledge in regulations and health finance related to management of inpatient care.

Excellent organizational skills, detail-oriented, a self-starter, possess critical thinking skills and be able to set priorities and function as part of a team as well as independently.
Commitment to working in a team environment and maintaining confidentiality as needed.
Excellent verbal and written communication skills including the ability to communicate effectively with various audiences.
Excellent interpersonal skills with the ability to manage sensitive and confidential situations with tact, professionalism, and diplomacy.
Possess and maintain computer skills to include working knowledge of Microsoft Office Suite and ability to learn other software as needed.

Job Specific Duties:

Exhibits an understanding of the importance of public relations, develops strong relationships and collaborates with external providers. These include, but are not limited to; long term care facilities, skilled nursing facilities, adult homes, and homeless shelters. Provides safe discharge options for patients discharging from the hospital with a variety of healthcare needs and available socioeconomic resources.
Implements and studies the case management model and structure to achieve appropriate length of stay resource utilization and clinical quality care for patients across the healthcare continuum. Responsible for designing and implementing improvement strategies for the assigned area(s) on an ongoing basis.
Oversees all activities related to referral and care coordination activities. Develops collaborative relationships with payers, maintains compliance with payer contracts and identifies opportunities for collaboration related to discharge planning.
Educates staff and the organization on an ongoing basis on national and regional trends and adjusts case management model to meet financial, regulatory and strategic goals for LHMC in collaboration with Administration. Maintains consistent processes to ensure organizational compliance with the Centers for Medicare and Medicaid Services (CMS) regulations, to include timely delivery of documents informing patients of their rights and responsibilities.
Develops annual budget with Administration for areas of responsibility and works with staff to implement the budget. Assures measures for staying within budgetary parameters.
Studies, evaluates, selects and implements systems that promote care planning prior to or on admit including pre-admission, admission, concurrent discharge and post discharge care expectations. Evaluates effectiveness and leads the assigned area(s) towards system wide improvement efforts to provide an effective transition structure across the healthcare continuum.
Collaborates and maintains strong working, effective relationships with the multi-disciplinary health care team to ensure seamless and safe transitions of care from the acute or emergency department setting.
Oversees and reports metrics to monitor effectiveness of discharge planning strategies. Ensures that patients, physician and staff needs are met in a timely manner and the organization is compliant with Federal/State requirements and insurance contracts/regulations.
Oversees the process for patients admitted to the assigned area(s) for social work and discharge plan needs. Continues ongoing assessment and interventions as needed to ensure appropriate, safe and timely discharges.
Oversees Spiritual Care for the organization. Works with the Spiritual Care department to recruit, provide training, and schedule volunteer and PRN chaplains in order to ensure Spiritual Care coverage 24/7.
Establishes and holds self and others accountable to foster a supportive work environment and strong desire to meet patient satisfaction goals, by delivering consistent, quality care and discharge planning.

Logan Health operates 24 hours per day, seven days per week. Schedules are set to accommodate the requirements of the position and the needs of the organization and may be adjusted as needed.
