UWMC Lung Post Transplant Services has an outstanding opportunity for a full-time day shift
PROGRAM COORDINATOR to coordinate the operation of a specialized or technical program: post lung transplant program. This person will coordinate and schedule extensive diagnostic tests and consultative appointments necessary for transplant evaluation, including independently prioritizing the needs of program clients, and coordinates pre transplant ambulatory (outpatient) continuum of care for lung transplant patients:
Primary Job Responsibilities:
Interact with providers, nurses and other team members to schedule all diagnostic tests as ordered by providers.
Report patient and/or family/caregiver concerns regarding medical, social, financial, psychological or cultural needs to appropriate transplant team member in order to facilitate the planning, personal comfort, or safety for the planned test or procedure.
Prepares schedule for diagnostic tests and clinic, plus interacts with patient and family/caregiver to provide necessary program instructions and to discuss itinerary of the diagnostic tests, consultative appointments, pre-test patient preparation instructions, and answer questions.
Responsible for facilitating pre transplant ambulatory continuum of care steps including initial lung transplant appointment, transplant evaluation and waiting list phases of care including: test, procedure and appointment scheduling, outgoing referrals and coordination of care with PCP/pulmonologists’ offices, and completion of missing outside provider documentation.
Manage referrals by entering referrals, providing records, and following up to ensure completion of referrals.
Coordinate the receipt and distribution to appropriate team member(s) of required items including but not limited to: patient forms, medical records, lab and diagnostic results from UWMC, primary care physicians’ offices, pulmonologists’ offices and other medical facilities.
Receive outside laboratory results on lung transplant patients undergoing serial laboratory monitoring per their treatment plan.
In preparation for return clinic visits, ensures that all appropriate medical records including but not limited to clinic notes/consultation reports, lab reports, radiology results, and pathology reports are obtained and provided for team members.
Maintain transplant charts in up-to-date and organized fashion including sending records for scanning into EMR, filing reports and clinical notes, and filing correspondence and regulatory documents.
Promote the program with outside organizations
Directs the work of others, such as student assistants or volunteers.
Minimum Qualifications:
High school graduation or equivalent AND two years of experience in a program specialty OR equivalent education/experience.
Additional Requirements include ability to effectively communicate in English, both verbally and in writing; computer knowledge including Word. Data entry experience; proficiency in word processing; ability to work effectively in a multi-disciplinary team environment; and excellent interpersonal skills and customer service skills. Need to demonstrate skills and experience in coordinating an efficient volume of patient appointment scheduling. Must be a multi-tasker who can prioritize work on a daily basis to meet patient needs and program deadlines.
