Job Summary:
As a member of Interdisciplinary Team, the Home Care Nurse works under the general direction of the Patient Care Coordinator and the attending physician/medical director to provide for skilled nursing assessment, planning, and implementation of the plan of care in order to maximize the comfort of terminally ill patients and their families.
Qualifications:
Graduation from an approved school of nursing, current registration in the State of Operation.
Minimum of two years experience in medical-surgical, oncology or community health nursing or equivalent.
Demonstrated ability to assist with and respond to the needs of terminally ill patients in the home setting.
Excellent assessment skills, communication skills, and ability to establish therapeutic interpersonal relationships with patients and caregivers.
Responsibilities:
Assume primary responsibility for a patient/family caseload that includes the assessing, planning, implementing, and evaluating phases of the nursing process.
Initiate communication with attending physician, other hospice staff members, and other agencies as needed to coordinate optimal care and use of resources for the patient/family.
Maintain regular communication with the Patient Care Coordinator to review caseload.
Maintain regular communication with the attending physician concerning patient/family caseload.
Obtain data on physical, psychological, social, and spiritual factors that may influence patient/family health status and incorporate that data into the plan of care.
State actions and goals accurately and clearly and reflect changes as they occur.
Seek input from other team members regarding the patient plan of care.
Accept responsibility for coordinating physical care of the patient by teaching primary caregivers, volunteers, and employed caregivers, or by providing direct care as appropriate.
Meet regularly with hospice team to review problems or unique issues from caseloads, share professional support, and exchange feedback aimed toward enhancing professional growth.
Attend patient conferences and facilitate discussion of issues from caseload for full staff discussion, consultation and evaluation.
Inform the Patient Care Coordinator of possibility for potentially problematic patient/family issues.
Provide appropriate support at time of death and period of bereavement.
Participate in orientation and in-service training programs for professional staff.
Demonstrate familiarity with policies of Assured Hospice Care and the rules and regulations of State and Federal bodies to aid in determining policies.
Take weekend and weekday on-call as needed or as assigned.
Participate in providing continuous care to patients as needed or as assigned.
Maintain consistent and regular attendance with few, if any, absences.
Accept other assignments as appropriate.
