Job Description :
Company in Detroit is looking for Social Workers for a full time hybrid position- going out into the Detroit and Wayne County community and working in an office setting
The job is a rewarding position where you are able to provide much needed assistance to the geriatric population in the Detroit area who need help the most!
Hours: 8:30am – 5:00pm Monday- Friday
Job Purpose:
Develops and completes participant’s care plan; establishes goals and directives of the care plans, and services to be utilized. Maintains regular, ongoing contact with participants, caregivers, and physicians. Conducts reassessments and monitors participant progress. Completes and submits timely paperwork and reports.
Job Duties include but are not limited to:
Educates participant, caregiver and service providers regarding the roles and responsibilities of each party, assuring that services are provided as documented in the care plan
Continues to evaluate the appropriateness of the service plan, adjusting it as needed
Monitors participant progress in relation to goals and objectives listed in the care plan
Interfaces with community resources as needed to support participants.
Produces timely and accurate commuter entry reports
Participates in training, supervisory meetings, in-services, and continuing education opportunities
Qualifications:
MUST BE EITHER LLBSW, LBSW, LLMSW, or LMSW
Valid Michigan Drivers License and car insurance and access to a vehicle is required
Valid Social Worker’s License in the State of Michigan
Experience:
Two-three years of experience completing intakes, pre-screening, and/or assessments, preferably in a health care or human service setting serving the elderly and/or disabled population
Knowledge of Medicaid and Medicare regulations
Ability to complete assessments using computerized screening tools and/or MS office
This is a temporary to permanent position.
Pay: $20.00 – $35.00 per hour based on education and experience
